For your convenience, we accept Care Credit, Visa, MasterCard, and Discover. Also, personal checks are accepted at our discretion. We deliver the finest care to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance.

We participate with most insurance plans, so please call our office to confirm our participation with your plan.
New Patient Forms
Prior to your first appointment, new patients are required to provide our practice with details such as name and address, medical history, payment responsibility, etc. This information is necessary for staff to maintain accurate, up-to-date dental records. If you have had a full set of X-rays or a panoramic X-ray within the last 3 years, please obtain a copy for our office and bring it with you on your first visit.
We know that your time is valuable. Therefore, we offer the option to reduce the time spent at our office simply by downloading, printing, and filling in the form at home. You can either bring it with you on the day of your appointment or email the completed form to dr.cucchiara@gmail.com
If you have questions regarding the information requested, please go ahead and provide as many details as possible — then on the day of your visit our staff will help you finish anything incomplete.
