What to do before your first visit 

Prior to your first appointment with the doctor, new patients are required to provide our practice with details such as name and address, medical history, payment responsibility, etc. This information is necessary for staff to maintain accurate, up-to-date dental records. If you have had a full set of X-rays or a panoramic X-ray within the last 3 years, please obtain a copy for our office and bring it with you at your first visit.

Your time is valuable and you can reduce the time spent at our office simply by downloading, printing and filling it in at home! You can either bring the form with you on the day of your appointment or email the completed form to While we suggest you print and fill in our registration forms before your appointment date, it’s just an optional convenience. You can of course complete the forms here at our office if you like — on the day you come in.

Financial Policy

For your convenience we accept Care Credit, Visa, MasterCard and Discover. Also, personal checks are accepted at our discretion. We deliver the finest care to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance.
